Free Sports Physicals

In order to play sports at New Smyrna Beach High School, all athletes must have a current physical and all other forms on file with the school. This Saturday, April 21st, Bert Fish Medical Center will perform sports physicals for free at the high school gym from 8am-1pm. You must bring the signed consent form and fill out pages 2 & 3 prior to your arrival at the school. You can pick up the forms at the high school athletic office or from your coach.
